Thank you for choosing Rebel Beauty Studio for your cosmetic procedure.

Prior to your appointment you MUST review the

Pre-Care instructions, client consent, and medical history forms.

AM I A GOOD CANDIDATE?

Before you are able to book an appointment, it is important for you, the client, to assess your own health to determine if you are a good candidate for the preferred procedure. Please review the following questions below to see if you are a good candidate. If you would like to talk about your condition in private you can always call me directly if you have any concerns.

  • Are you pregnant or breastfeeding? By law I cannot tattoo on anyone that is pregnant or breastfeeding.

  • ​Do you have irritation of the skin of any kind such as rosacea, blemishes, pimples, Psoriasis on or around the area of procedure at the time procedure?

  • Do you tan on a regular basis?

  • Are you on any Retinols?  You must be off  retinols a minimum of 6 month before appointment. If you are on Accutane I can not tattoo you at this time.​

  • Are you on any lash or hair enhancement serums? You must be off hair growth enhancement for a minimum of 6 months for any eyeliner procedure​

  • Have you had a Botox within the last 2 weeks of the procedure? You must wait 3 weeks after a Botox treatment before a procedure. 

  • Are you undergoing chemotherapy or have any kind of compromised immune system? If so, please consult your doctor and have ready a Drs. release prior to your procedure.

  • Do you have any virus, flu, sickness, or cold? I understand that sickness happens. If you are sick we can reschedule at a better time when your immune system is not compromised.

  • Do you have diabetes? If so please consult your doctor prior to booking.

  • Do you have any serious heart problems or complications. If so please consult your doctor prior to bookings.

  • Are you are on prescription drugs? Surprisingly, some prescriptions can be a contradiction with certain pigments used in permanent cosmetics. Please consult your doctor before booking to make sure you are cleared. 

  • Are you are allergic to Lidocaine, Tertracaine, or Epinephrine? These are ingredients commonly used in numbing agents. If I can’t numb you, I cannot work on you safely.​

  • Lastly, is there anything in our health history that inhibits healing, excessive bleeding, or a compromised immune system.

PRE CARE  INSTRUCTIONS

Before your Procedure

  • Don’t drink heavily the night before your procedure; it could thin your blood for the next day.

  • You cannot be under the influence of alcohol or drugs at the time of your tattoo appointment.

  • Drink lots of water – be well hydrated.

  • Get plenty of sleep the night before.

  • If you have any medical condition that requires a doctor’s note beforehand, make sure you have the necessary physician’s authorization form.

  • If your skin is sunburned on the area to be worked on – reschedule your appointment.

  • Have your ID ready and available  - it must be State/Federal/Military/Government issued.

  • Consider packing a small bag with bottled water or juice, snacks, a pillow, a small clean blanket.

  • Consider wearing comfortable clothing.  

  • Eat within 2 hours of your cosmetic/tattoo procedure.

  • Avoid caffeinated or energy drinks right before your procedure appointment.

  • Avoid aspirin and/or ibuprofen products for 24-48 hours before your appointment, unless your physician requires it.

  • You will have to fill out paperwork with medical history information.​

During your Cosmetic

  • Your specific procedure will take time. Be prepared to sit for a period of time for adequate numbing and lying on a cosmetic procedure bed without moving around during your procedure.

  • You need to stay calm. Let your artist know if you’re nervous or scared, and they can often help by explaining the process to you. You should try to keep your body relaxed, breathe steadily and slowly.

  • Inform your artist if you’re feeling nauseous or dizzy.

  • Pay attention to the instructions your technician gives you during the procedure.

  • Inform your artist if you need to take a short break.

  • If you need to move, stretch, cough, etc. – warn your technician first.

  • Expect to feel tired after your procedure.

The following are some helpful tips about the pre permanent

cosmetics procedure, and how to prepare. Please note, if you are on any prescription retinols, you must be free of using them for a minimum of 3 months before any procedure. For eyeliner, you must be free from use of any lash enhancement serums for a minimum of 6 months

CANCELLATION POLICY

If you need to reschedule or cancel I require a minimum of 2 days notice (48 hours). I’m happy to reschedule, but please respect that I have a very tight schedule. If you cancel last minute you will be charged an additional non refundable reservation fee and will have to reschedule.  To be respectful to other clients patiently waiting for their appoint please understand there are no exceptions around cancelation policies.

PAYMENTS

I accept cash, card, and check. A non refundable booking fee of $50 is due at time of booking that will be deducted from your total cost.
