Client Consent and Medical Forms

Are you ready for your appointment!!! The exciting day will be here before you know it. Please make sure you have printed the following paperwork and filled it out to bring to your appointment.  You will also need a government issued ID, drivers license, or passport. You must be 18 years or older. If you are pregnant, nursing, or plan to become pregnant you will need to reschedule.
